You need the information below to complete the accident report request form:

  • Date of accident,
  • Time of accident,
  • Location of the accident,
  • Involved vehicles,
  • Involved parties,
  • Insurance coverage information,
  • Description of damage,
  • Description of injuries,
  • Investigating officer,
  • Summary of the accident,
  • Names and addresses of any witnesses, and
  • Contact phone number.

You can attach a plain sheet of paper to include any information that does not fit on the request form.
